Self-treatment has done nothing to slow the progress of this lesion which the patient thought might be an insect bite at first. Your Dx?

Gina presents with an advanced leg ulcer that 5 or 6 weeks ago began as what she presumed was in insect bite. Despite home treatment with antibiotic ointment it has enlarged and become painful.
