Sometimes trying to locate a tiny shard of glass in the sole of the foot is like looking for a needle in a haystack. Even after a radiograph confirms the presence of the glass, it still may be difficult to find-especially in the thicker heel pad. A solution that I have found helpful in patients with pinpoint tenderness is to perform a punch biopsy directly over the tender area. When you extract the core, the piece of glass is usually inside. Depending on the size of the punch, often no sutures are required. When a larger tissue sample is removed, making an elliptical incision facilitates suturing.
